Under-eye bags or puffiness

The under-eye area may look full because of lower-eyelid fat pads, fluid retention, malar bags or a combination. Distinguishing them matters because treatment differs.
Signs of prominent fat pads
Fat pads usually create stable bulges directly beneath the lower eyelid. They change little during the day and may be inherited.
Signs of puffiness
Fluid-related swelling is softer and fluctuates, often worsening in the morning or after salt, alcohol, poor sleep or allergy. Persistent new swelling should not automatically be treated as cosmetic.
What the surgeon assesses
Skin quality, fat position, eyelid tone, the tear trough, cheek contour and symmetry are assessed together. A nearby hollow may make a bulge look larger.
When surgery helps
Lower blepharoplasty may reposition or reduce fat and address skin when indicated. It does not treat systemic fluid retention.

Do cold compresses work?
They may temporarily reduce puffiness but cannot remove fat pads.
Why do bags look stronger in photographs?
Directional light accentuates the transition between a bulge and the tear trough.
Which symptoms need attention?
Sudden one-sided swelling, pain, marked redness, breathlessness or visual disturbance requires medical care.
