In the first weeks after plastic surgery, the right and left sides rarely look identical. Swelling resolves unevenly, muscle tone differs and tissues settle at different rates. This does not make every asymmetry normal: a rapid change on one side may indicate a haematoma, seroma, infection or another problem.

I am Anna Brodska. Here I explain how to monitor change without hourly mirror checks, how to take useful photographs and when not to wait for a routine appointment.

If one side rapidly enlarges, becomes hard, hot, substantially more painful or changes colour, contact the surgical team immediately. Call emergency services for breathlessness, fainting or active heavy bleeding.

Why sides heal differently

The body is asymmetric before surgery: brows, breasts, ribs, pelvis and fat thickness differ naturally. Surgery adds tissue trauma, sleeping position, dominant-arm activity, compression and gravity.

One photograph cannot show speed. The trend and accompanying symptoms determine urgency.

After breast surgery, one side may sit higher because of muscle spasm and swelling. After a facelift, expression and sleep affect the difference. After liposuction, swelling and garment creases alter contours. Context matters.

How to monitor at home

  1. Take photographs only at the interval advised—often every few days or weekly.
  2. Use the same place, neutral light, distance and camera height.
  3. Stand or sit straight without deliberately tensing muscles.
  4. Capture an overview and a detail; avoid a close wide-angle selfie.
  5. Record pain, temperature, tension and when the change began.
  6. Do not press tissues to “test a pocket” or move an implant.
  7. For a rapid change, send a sequence with symptoms, not a single unexplained image.

Different camera heights and selfie lenses create false asymmetry. The mirror also looks different after showering, in the evening and immediately after removing compression.

Helpful actions

  • Wear the prescribed garment in the correct position.
  • Sleep and elevate the head or limbs as directed.
  • Walk and increase activity gradually.
  • Take only prescribed medication.
  • Avoid one-sided loads and heavy bags.
  • Attend follow-ups even when pain has resolved.
  • Keep a brief recovery log.

What not to do

  • Do not press or massage one side without permission.
  • Do not tighten compression independently.
  • Do not apply heat to a new swelling.
  • Do not compare early healing with retouched photographs.
  • Do not repeatedly measure contours throughout the day.
  • Do not start a device treatment to “even things out.”
  • Do not reposition postoperative bands yourself.
  • Do not wait when asymmetry is rapidly worsening.

When can the result be judged?

Timing varies. Facial swelling alters contours for weeks, breast implants and tissues settle over months, and body contour evolves as swelling and scars remodel.

I separate three checkpoints:

  • early safety: bleeding, infection and tissue perfusion;
  • intermediate progress: swelling and tissue stabilisation;
  • long-term result: shape after major healing and scar maturation.

Revision discussion is often premature early on, but suspected complications must not be dismissed as “just swelling.”

Reasons for urgent review

Contact the team promptly for:

  • rapid enlargement on one side;
  • severe one-sided pain or hard tension;
  • active fresh bleeding through a dressing;
  • spreading redness, heat or purulent drainage;
  • sudden skin-colour change;
  • new marked deformity after movement or trauma;
  • fever and systemic deterioration;
  • breathlessness, chest pain or fainting.

Frequently asked questions

Why is one breast higher after augmentation?

Baseline asymmetry, different muscle spasm, swelling and pocket adaptation can contribute. This is assessed at follow-up. New rapid elevation with pain and tension requires prompt contact.

Can massage correct asymmetry?

Not on your own. Some techniques have indications, while others may displace tissue or worsen bleeding. Diagnosis comes first.

Why does my face differ morning and evening?

Sleep, posture, activity and gravity alter fluid distribution. A longer trend on standardised photographs is more informative.

When is asymmetry final?

There is no universal deadline. Final assessment follows resolution of major swelling and tissue stabilisation for your specific procedure.

Key message

Temporary asymmetry is often part of healing, but speed, pain, firmness, skin colour, drainage and general health determine safety. Monitor in a standardised way, never manipulate tissues to correct them yourself, and do not judge a final result in the first weeks. A rapid one-sided change needs review.
