A raised temperature after plastic surgery does not have one universal cause. In the first hours, inflammation, medicines, dehydration and measurement conditions may contribute. Later, wound, respiratory or urinary infection, a medicine reaction and rarer serious complications also need consideration.

I never assess the number alone. The postoperative day, repeated readings, chills, pulse, breathing, pain, wound appearance, urination and ability to drink all matter. A temperature cannot diagnose the cause, but a cluster of symptoms may demand urgent action.

Follow the threshold and contact pathway in your own discharge instructions. If you feel acutely unwell, do not wait for the thermometer to reach a particular number.

What may cause a temperature

These are clues, not a self-diagnosis. More than one factor can occur together.

How to measure usefully

Skin warmth beside the wound is not a substitute for body temperature. Assess it separately with colour, swelling, pain and discharge.

Information the surgeon needs

  • postoperative day and exact time;
  • highest and latest readings and measurement method;
  • chills, sweating, weakness or confusion;
  • changes in pain, swelling and wound appearance;
  • cough, breathlessness or chest pain;
  • urinary symptoms;
  • every medicine taken and the last dose time;
  • fluid intake and any vomiting.

A wound photograph can support remote triage, but does not replace examination when systemic symptoms are present.

A home assessment sequence

  1. Rest quietly and repeat the reading with a reliable thermometer.
  2. Record number, time, method and recent medication.
  3. Check breathing, alertness, pulse, pain, wound and urination.
  4. Compare with the personal discharge instructions and clinic threshold.
  5. Contact the surgical team when the threshold is reached or other concerning signs occur.
  6. Do not start leftover antibiotics or hide symptoms with several medicines.
  7. Call emergency services for emergency symptoms rather than waiting for a message reply.

When emergency care is needed

  • breathing difficulty, chest pain or blue lips;
  • fainting, severe confusion or inability to wake;
  • rapid deterioration, severe shaking chills or profound weakness;
  • tongue or facial swelling after medicine;
  • severe increasing pain or tense one-sided swelling;
  • repeated vomiting with inability to drink;
  • very little or no urine.

What not to do

  • do not overheat under blankets during chills;
  • do not wipe the body with alcohol or ice water;
  • do not start an antibiotic “just in case”;
  • do not exceed paracetamol limits or duplicate it in combination products;
  • do not conceal fever because you fear an extra appointment;
  • do not rely on a photograph or chat alone during deterioration.

Frequently asked questions

Is 37.5°C normal?

A mild early rise can occur, but the number needs context: timing, measurement method and symptoms. Follow your clinic’s instruction.

Does fever always mean wound infection?

No. Causes vary, and a wound infection may sometimes be local without high fever. Assess both the wound and whole-body condition.

Can I take a fever reducer immediately?

Use only the medicine and dose authorised by the team. Record the reading first and avoid duplicate ingredients.

Should I write or call?

Call when the clinic threshold is reached or fever accompanies new pain, redness or discharge. Breathlessness, fainting or rapid deterioration requires emergency care.

The central point

Postoperative temperature is a prompt for structured assessment, not an automatic diagnosis. I ask patients to document it, check the wound and accompanying symptoms, and follow their individual discharge pathway. Feeling severely unwell matters more than waiting for a particular number.
